When the muscles surrounding it become overloaded through habitual clenching, bruxism (teeth grinding), stress holding or postural strain, the result is a complex pattern of jaw pain, facial tension, headache, ear ache, neck stiffness and even referred pain into the shoulders and upper back.
TMJ massage at Pirom Cromwell is a 15-minute targeted add-on to your regular massage session — focused specifically on the muscles, fascia and soft tissue structures that directly drive TMJ dysfunction and jaw tension. It is not a standalone facial treatment. It is precise, clinically informed soft tissue work applied to the structures that most people never have properly addressed.
Our approach combines intraoral-adjacent external work on the masseter, temporalis, pterygoids (external approach), digastric and suboccipital muscles — releasing the layered tension patterns that maintain TMJ dysfunction long after the original cause has resolved.
The primary jaw-closing muscle — heavily loaded in grinders and clenchers. Palpable at the cheekbone, it becomes hypertrophied and tender in chronic TMJ sufferers. Direct massage and trigger point release of the masseter produces immediate reduction in jaw tension and often resolves referred ear and cheek pain.
A broad fan-shaped muscle covering the temporal region of the skull — responsible for jaw elevation and retraction. Temporalis tension is a primary driver of temporal headaches, eye socket pain and the sensation of pressure around the head. Release of the temporalis can immediately reduce tension headache intensity.
The medial and lateral pterygoid muscles are deep jaw muscles involved in jaw opening, protrusion and lateral movement. External pterygoid tension contributes to jaw clicking, deviation on opening and deep TMJ joint pain. We access these muscles through careful external palpation at the posterior ramus of the mandible.
The four suboccipital muscles at the base of the skull have a well-documented clinical relationship with TMJ dysfunction — tension here maintains the forward head posture that loads the jaw, and suboccipital tightness often produces referred pain mimicking TMJ symptoms. Release here is essential for lasting jaw relief.
The digastric and suprahyoid muscles beneath the chin assist in jaw opening and hyoid movement. Tightness here contributes to swallowing difficulties, throat tension and restricted jaw opening range — commonly overlooked in TMJ treatment but addressed in our approach.
The superficial fascial layer of the face connects across the forehead, cheeks and jaw — tension patterns in one area perpetuate holding in others. Broad fascial release across the face completes the session, reducing the generalised sense of facial tightness and pressure that TMJ sufferers describe.
